The Family Support Waiver is a Nebraska Medicaid waiver for children and youth with developmental disabilities, from birth to age 21. It helps families access supports like day services and community integration. It's the waiver our children's day services are offered through — your Service Coordinator can confirm eligibility and which services are authorized.

Yes. Every team member is background-checked and fully trained before working with anyone we serve, and we operate in compliance with Nebraska DHHS requirements. We also have a registered nurse on staff to support health needs and medication oversight.
Yes. Schedules are flexible — full days or select days both work. We'll build a schedule around the individual's needs and authorized services.
Just the everyday essentials: a lunch, any daytime medications (in their original labeled containers), and a change of clothes. We'll let you know if anything else is needed for a specific activity or outing.

Our day program runs Monday through Friday during daytime hours, following a structured weekly rhythm of wellness, community outings, life skills, and recreation. The office is open Monday–Friday, 8:30am–4:30pm. Residential Shared Living is provided in the home year-round.
With an approved Nebraska DD waiver, services are funded through the waiver (Medicaid) — there's no direct cost to your family. Your Service Coordinator handles the authorization. We accept the Nebraska Family Support Waiver (FSW) for day services (children and youth, birth–21), and we serve adults through the Developmental Disabilities Adult Day (DDAD) and Comprehensive Developmental Disabilities (CDD) waivers for day services and residential Shared Living. A participant can be on only one waiver at a time — your Service Coordinator can confirm which one applies.
Referrals start with the Service Coordinator. Let us know you'd like TLC at Home as the provider, then email or fax the referral documents. We follow up, complete the referral form with you, confirm the authorized services, and match the individual with the right staff so we're ready to begin as soon as everything's authorized.

On our end, we're ready to move quickly — we have immediate openings. The overall timing depends on your Service Coordinator, funding authorization, and in some cases the guardian, so it varies from person to person. As soon as services are authorized, we match the individual with the right staff and get started.
Yes — we currently have openings for both Family Support Waiver day services and adult Shared Living across Omaha and La Vista. There's no waitlist; reach out and we'll get the process moving.
Shared Living is a residential option where a trained provider welcomes an adult with a developmental disability into their home, offering a supportive, family-style environment that promotes independence, community life, and a real sense of belonging — rather than a facility setting.
We'd love to talk. Shared Living Providers open their home to an adult with a developmental disability, with ongoing support from our team every step of the way. Required training varies based on the individual's needs and provider requirements, and may include areas such as crisis prevention, First Aid/CPR, medication-related training, documentation, safety, and person-centered support. TLC provides MANDT training at no cost to the provider and will guide applicants through any additional training requirements. This is a contracted role: providers are compensated through the individual's Nebraska DD waiver once they are matched with someone to support, and we'll walk you through how it all works. At TLC at Home, Shared Living Providers are never on their own. From the initial application through placement and beyond, our team provides ongoing guidance, training, and support every step of the way. We help providers understand and implement each individual's Person-Centered Plan (PCP) / Individual Support Plan (ISP), safety and nursing protocols, documentation requirements, medication-related responsibilities, community inclusion, and other individualized support needs. We also provide ongoing coaching, resources, and assistance to help providers feel confident while creating a safe, stable, and meaningful home environment for the individual they support.
